Not necessarily. The answer to this question is always dependent on individual conditions, but for people with type 2 diabetes, insulin treatment is often avoidable.

In case of type 1 diabetes, diet is unfortunately not a viable option, insulin therapy is necessary. Type 2 diabetes or insulin resistance can be well managed with a healthy lifestyle. 

Some laboratory parameters are usually needed in the diagnosis of metabolic disorders such as OGTT (oral glucose tolerance test) with insulin and commonly lipid panels.
